Asking for a copy or a transfer
You can ask for a copy of your eye care records, or ask us to send them to another eye care provider. Please make your request in writing, or call us and we will explain how. We may ask you to confirm who you are before we release records.
We will respond within 30 days. If we need more time, we will tell you in writing, and the extension will be no more than 30 more days. If there is a fee for copies, we will tell you the amount before we prepare them.
