Privacy policy

How we collect, use, share and protect your personal health information, and how to reach us about your privacy.

Our commitment

Dr. Dar Eye Care is the practice of Dr. Dar Lenarcik & Associates, optometrists in London, Ontario. We are a health information custodian under Ontario's Personal Health Information Protection Act, 2004 (PHIPA). This statement describes our information practices: what we collect, why we collect it, how we protect it and your rights.

What we collect

We collect only the information we need to provide your eye care and run our clinic, such as:

  • Your name, date of birth and contact details
  • Your health card number and insurance details, so OHIP or your insurer can pay for covered services
  • Your health history, eye exam results, prescriptions and notes from your visits
  • Your appointment requests and the messages you send us

On this website

When you request an appointment, we collect the name, date of birth and contact details of each person coming, the exams and times you prefer, and anything you choose to add, such as accessibility needs. We do not ask for your health card number online. When you use our contact form, we collect your name, email, phone number (if you give it) and your message.

How we use and share it

We use and share your information to:

  • Provide your eye care, including referrals to other health care providers when you need them
  • Book, confirm and remind you about your appointments
  • Bill OHIP, other government programs or your insurer for covered services
  • Meet our legal and professional duties, such as keeping the records the College of Optometrists of Ontario requires

We do not sell your information, and we do not use it for marketing unless you have clearly agreed. We share it for other reasons only with your consent or where the law requires or allows it.

Texts and emails

If you agree, we send texts and emails about your requests and appointments, such as confirmations and reminders. Regular texts and emails are not fully secure, so we keep them to scheduling details and never include test results or other health details.

You can stop these messages at any time: reply STOP to a text, use the unsubscribe link in an email, or call us.

How we protect it

We use physical, administrative and technical safeguards, for example:

  • Only staff who need your information for their work can see it
  • Staff sign in to our booking system with a password and a one-time code
  • Information is encrypted when it travels over the internet and when it is stored
  • Our booking system records who looked at or changed booking information

If your information is lost, stolen or seen by someone who should not see it, we will tell you at the first reasonable opportunity.

Where your information is stored

Our website and online booking system are hosted in Canada. Some companies that help us deliver messages may process information outside Canada: for example, our text message provider processes messages in the United States. We limit what these companies receive and require them to protect it.

How long we keep it

Ontario rules for optometrists require us to keep patient health records for at least 10 years after your last visit, or for 10 years after your 18th birthday if you were under 18 at your last visit. Appointment requests and messages are kept with your records. When records are no longer needed, we dispose of them securely.

Seeing and correcting your records

You have the right to see or get a copy of your records, and to ask us to correct information that is wrong or incomplete. Please send your request in writing to our privacy contact (below). We may ask you to confirm who you are.

We will respond within 30 days. If we need more time, we will tell you in writing, and the extension will be no more than 30 more days. If there is a fee for copies, we will tell you the amount first.

You can withdraw your consent to how we use or share your information by telling us. This does not affect information already used or shared, or uses the law requires.
